CRBA Appointments in Berlin

How to schedule an appointment in Berlin:

In order to provide better service to our clients, as of April 1, 2019, all appointments for Consular Reports of Birth Abroad (CRBA) and adult derivative citizenship applications (first time applicants over the age of 18) at the U.S. Embassy in Berlin, will be made only through a mail-in appointment system.

To request an appointment:

  1. Print the CRBA checklist
  2. Gather all required documents on the checklist, including the required application forms. Please utilize the passport form filler found here to complete the DS-11. The DS-2029 form is found here.
  3. Send the following to the U.S. Embassy mailing address provided below:
    • Completed CRBA checklist;
    • Original of completed forms and one photocopy of required documents as outlined on the checklist (#1-11);

Do NOT send originals of your identification/civil/legal documents. To ensure the safety and speediness of your application we recommend that you use a courier service so that you can track your package. Drop-off requests will not be accepted.

Once your packet is received, our staff will contact you via phone or email to schedule an appointment, and if needed, will share a list of items you are missing. Please ensure that your contact information on the application is accurate and current.

Please be advised that applicants who fail to show up for their scheduled appointments will need to restart the appointment request process.
